    In temeiul prevederilor art. 10 alin. (6) din Hotararea Guvernului nr. 1.574/2003 privind organizarea si functionarea Ministerului Finantelor Publice si a Agentiei Nationale de Administrare Fiscala, cu modificarile si completarile ulterioare,
    in baza dispozitiilor art. 196 alin. (2) din Ordonanta Guvernului nr. 92/2003 privind Codul de procedura fiscala, republicata,
    avand in vedere prevederile art. 71 din Ordonanta Guvernului nr. 92/2003, republicata,

    ministrul finantelor publice emite urmatorul ordin:

    Art. 1
    Se aproba modelul si continutul formularului "Declaratie privind sediile secundare", cod M.F.P. 14.13.01.99/b, prevazut in anexa nr. 1.
    Art. 2
    Se aproba caracteristicile de tiparire, modul de difuzare, de utilizare si de pastrare a formularului "Declaratie privind sediile secundare", cod M.F.P. 14.13.01.99/b, prevazute in anexa nr. 2.
    Art. 3
    Anexele nr. 1 si 2 fac parte integranta din prezentul ordin.
    Art. 4
    Declaratia privind sediile secundare se depune la organul fiscal in a carui raza teritoriala se afla domiciliul fiscal al contribuabilului care le infiinteaza.
    Art. 5
    Directia generala de gestiune a impozitelor si contributiilor, Directia generala de colectare a creantelor bugetare, Directia generala a tehnologiei informatiei, directiile generale ale finantelor publice, administratiile finantelor publice ale sectoarelor municipiului Bucuresti si Directia generala de administrare a marilor contribuabili vor duce la indeplinire prevederile prezentului ordin.
    Art. 6
    Prezentul ordin va fi publicat in Monitorul Oficial al Romaniei, Partea I.
